The Wells Score is a clinical prediction rule for estimating the probability of deep vein thrombosis (DVT).

Active cancer (treatment ongoing, within 6 months, or palliative)

Paralysis, paresis, or recent plaster immobilization of the lower extremities

---

Recently bedridden for ≥3 days or major surgery within 12 weeks requiring general or regional anesthesia

---

Localized tenderness along the distribution of the deep venous system

---

Entire leg swollen

---

Calf swelling ≥3 cm compared to the asymptomatic leg (measured 10 cm below tibial tuberosity)

---

Pitting edema confined to the symptomatic leg

---

Collateral superficial veins (non-varicose)

---

Previously documented DVT

---

Alternative diagnosis at least as likely as DVT

Interpretation of Scores

• 0 or less: Low probability of DVT
• 1-2: Moderate probability of DVT
• 3 or more: High probability of DVT
